We’re so glad your family is part of our community at Epiphany Lutheran Church! This form helps us prepare for a safe, fun, and meaningful year in Children, Youth, and Family Ministries. 

Please complete one form per household, including information for each child or youth participating in our programs. You’ll also find opportunities to get involved as a parent/guardian/significant adult, and important sections for permissions and release. Thank you for helping us stay connected and support your child’s faith journey!
